It is the policy of Brewton City Schools to use a transcript request form when individuals are unable to obtain the transcript in person.
This form must be completed in its entirety, notarized and returned along with a copy of photo ID to the central office by regular mail to: 811 Belleville Avenue, Brewton, Alabama 36426
Your Information will be released upon receipt of the authorization form.
